All four of the items below are required and must be completed to volunteer.

1. VOLUNTEER LIVE SCAN FINGERPRINT FORM - complete once

2. VIRTUS Online Safe Environment Training - repeat every 3 years

3. VOLUNTEER ACTIVITY WAIVER FORM - complete annually

4. VOLUNTEER TB SCREEN FORM - repeat every 4 years
